Our Intake Forms

AFTER you have contacted our office to schedule an appointment, someone will instruct you as to which forms to fill out to complete the intake process.

What to Expect at the Initial Evaluation

The initial evaluation is where we gather all the information we need to develop a plan of action for your child. This includes reviewing your intake paperwork, past assessments, and IEPs, and also discussing your concerns for your child. 

Plan for one adult to attend the 60-minute initial evaluation to provide information and collaborate on goals. As an important member of the team, you are encouraged to ask questions and be an active part of this process.

We use a variety of assessment methods to understand your child's strengths and challenges. However, we move at your child’s pace with a first visit goal of building trust and connection with your child for a positive experience. 

After the evaluation is complete, we will review our results with our recommendations for ongoing care. When findings of the evaluation reveal a need for therapy, we will get you scheduled for ongoing sessions based on the recommendations of your therapist.

What to Expect at Your Child’s Sessions

Each session is individualized to meet the needs and interests of your child to keep them engaged and comfortable while making progress and meeting goals. When possible, send your child in comfortable clothes that allow for lots of movement. 

We will keep you updated after each session and provide ongoing recommendations for home. 

Physician's Referral

For insurance reasons, a physician referral is needed for therapy services. Ask your child's doctor to fax a referral to our Easton office at 410-822-2963 or our Salisbury office at 443-978-7512.  If needed, we help guide you with this step.

Insurance Coverage for Pediatric Therapy

We accept most major insurance carriers and will verify your coverage so you know what to expect prior to your first visit. If you have a specific question about your insurance coverage, call us we’re happy to get that answered before starting therapy. 

Keeping Your Family Safe with Our Sickness and Covid-19 Policy

Here is what to expect for EACH visit:

 The therapist will meet you at your car and assist your child with entering the clinic.

  • Masks are optional.

  • Every person washes their hands upon entering the building. 

  • We clean every item after it is used by a child. 

  • Only one family member is allowed to attend along with your child. Unfortunately, siblings are not currently permitted to attend therapy sessions. 

  • If in doubt regarding potential illness or Covid exposure, please call to reschedule your appointment.
